A 75 yrs hypertension, asymptomatic female come for routine checkup. Few months ago she was admitted for AKD following Vomiting after that she has been advised following medicines (Tab Olmesartan 10mg OD, Iron Capsules OD, Tab Calcium and Vit D3 OD & Multivitamins Capsules daily). Now her Serum creatinine -2.29 mg/dl, BUN -42.66 mg/dl, Serum Uric Acid-7.02 mg/dl, Hb-8.2 gm/dl, Urine for Microalbumin- 59 mg/L, Na-139 meq/l, K- 6.59 meq/l, Cl-110 meq/l.

No pedal edema or SOB found. EGFR calculated as per Cockroft Formula is 104. What further evaluation required & management in this case?
