In a randomized trial of 30 patients with atrophic posterior mandibles, implant survival was similar for long i mplants with augmentation (89.7%), short implants (91.7%), and overdentures (85% ). Short implants showed the least crestal bone loss and favourable peri-implant tissue health, while overdentures had better plaque and gingival indices but greater bone loss. Patient satisfaction was highest with fixed restorations (short or long implants), especially for aesthetics and function, whereas overdentures scored better for surgical morbidity and maintenance. Overall, short implants provided the best balance between biological outcomes and patient satisfaction.
