A 31 yr old female pt has had the following obstetric history. She is currently in the interval period and contemplating surrogacy. !st pregnancy - 2012 - !st & 2nd trimester bleeding - on and off - Had PROM at 28 weeks with placenta previa. Conserved with bed rest, iv antibiotics for 3 weeks. LSCS at 31 weeks, birth wt 1700gms. Perinatal death after 36 hrs (cause?). Pt had high grade fever 1 week after LSCS.
2nd pregnancy- 2014 - conceived after IVF since HSG was s/o bilateral tubal block. Dichorionic diamniotic twins. PROM from one sac at 11 weeks with a spontaneous expulsion of that fetus at 13 weeks. cerclage done at 15 weeks for second twin, but again had PROM from second twin at 17 weeks, inevitable abortion, started oxytocin & terminated. The patient then had a diagnostic / operative scopy in 2014 with hysteroscopy tubal cannulation. Laparoscopy confirmed bilateral spill.…
