Cant doctors select their patients as the patients select their doctor

Am a diabetologist. Many a times I find patients not following the instructions though repeated patiently any number of times.

Instead of shouting at them to follow I ask them to consult someone else which gives me peace of mind. As the patients have the right to choose the doctors of their choice, why not the doctor?
