Case of acute and severe dyspnoea with gross edema of limbs diagnosis

An adult female, 26 who underwent LSCS 2 weeks back was brought with these symptoms of dyspnoea and severe limb edema. She had received transfusions thrice during pregnancy. Her creatinine was 1.7 mg% on discharge. GPE- BP140/80, P130/m, RR 34/m severly breathless. JVP elevated. RS diffuse crackles.CVS no murmers, no cardiomegaly. Spo2 84-86%, no calf tenderness, urea165, cr2.2 ABG hypoxiaemia with compensated metabolic acidosis with Lactate levels in range.

Was admitted in ICU, put on intermittent NIV. Reports during pregnancy. Positive for ANA, anti-ds DNA, anti- SM, anti SSA, anti cardiolipin antibodies. Also positive for lupus antibodies. Echo normal PAH, rest all normal, chest films given below. Kindly Suggest Diagnosis.
