Respected doctors, A 53years male known case of type 2 dm presented with weakness, fatigue, tiredness for 4 days.On examination patient was well oriented to time,place and person, Pulse-80/min, BP-126/82 mmhg, RR-16/min, Temp-98.6 F. Hydration status-Normal, CVS/RS- NAD. BSL-R-205, SPO2-98%. No history of vomiting, loose motions, Sun exposure, polyurea or polydypsia. Lab Showed- BSL-R-205, HB-11gms/dl,TLC-13200,Creat-1.46. Na-104, K--2.4.
He was started on 3% NS@ 20ML/HR and POTCHLOR 15ml three times a day.He was onTelma-H and stopped after admission. Repeated sodium and potasium done, but even after 3 days sodium is 122. He was also started on iv Gramocef 1gm/bd. Potassium is 3.2.What could be the cause? How to manage further??
