A lady delivered twins on 8 th Sept 2014,both were Normal weight,both had given Pentavalent Vaccine at 6 wks and 10 wks.After which lady shfted to our visiting village where third dose of pentavalent vaccine were given at 14 wks on 6 th 2015.As per ASHA(Accredited Social Health Actvist) one baby had URTI but family member didn't disclose at the time of immunization,same baby were given eggs liquid orally and also applied it on ches,may bet to relive from ? URTI.Baby was breast fed in evening and then later in night at 1:00 A.M.,No belching after breast fed at 1:00 A.M.,At appox 3:00 A.M. mother found baby dead.
