In a road accident while driving on two wheeler, 45 years female was hit by car . She sustained blunt abdominal injury. She presented with severe pallor, tachycardia & BP 80 systolic. USG & CT Abdomen with IV contrast showed Splenic rupture with severe parenchyma lacerations with active bleeding. There was moderate heamopertoneum with out any other injury. Energetic resuscitation brought BP up. Emergency Lapartomy was carried out. There was almost 1.5 lit of blood in peritoneum.
First of all Splenic Artery secured. Then spleen examined which was completely shattered & there was contributed bleeding so it was not possible to preserve it. Ultimately splenectomy was done. Splenic tissue were implemented in omentum. 6 units of blood were given. Vaccination to be done after two weeks. Any opinions on this?
