Healthy looking 60 year male with history of chronic gastric and hyperlipidemia came with continuous but low fever, weakness and decreased appetite and weight loss. His urine had very dark discoloration. He did not take any medicine except gelusil.. He didn't drink at all but used to smoke (1-2 packs/week).
Family history had cases of diabetes mellitus, hypertension, myocardial infarction, and stroke. Ultrasound and CT of abdomen showed dilated common bile duct, dilated pancreatic duct, and enlarged lymph nodes in the porta hepatis, peripancreatic and perigastric regions. Whats the diagnosis and treatment?
