40 yrs old lady presented with 6 months fever, wt.loss, diagnosed HIV and with lt.lz consolidation. Pt.was given Anti HIV drug and IV antibiotic. Her sypmtoms were improved but few hours of fever was there after 5 days of antibiotic.Then I refered her to HIV specialist. He stopped etroviral and stated AKT without evidence of Koch's. Pt.'s fever subsided but just after a week AKT have to be stopped due to vomiting. Then after again it was started by HIV specialist. Again after few days she presented with high grade fever(104degree), bilat.
knee artharlgia with high s.uric acid ~14mg/dl. I took his adviced on phone and he adviced to stop only pyrizinamide. I have stopped all AKT drug started iv antibiotic and pt. has improved. My question is whether should I continue AKT or not?
