Case presentation A 48-year-old female reported to the hospital with progressive breathlessness and cough with expectoration for the past six months. Medical history She was treated for pulmonary tuberculosis. Physical examination revealed: Multiple rashes and papules over the face, neck, and trunk Generalized lymphadenopathy involving cervical, axillary, and inguinal nodes bilaterally Complete hemogram examination revealed: Mild anemia (hemoglobin -10 gms) Mild thrombocytopenia (platelet count - 1,00,000 platelets/µL) Elevated leukocyte count (total leucocyte count - 61000/µL) Peripheral smear examination revealed: Atypical lymphoid cells, some had the typical cerebriform nuclei (Cover image, shown in black arrow)  A skin biopsy was performed due to multiple maculopapular lesions present over the face, neck, and trunk.
