Dear colleagues, This is a rare form of ectopic thyroid diagnosed by thyroid nuclear scan. First one is a big one in the midline isthmus area mimic like a thyroglossal cyst. But the swelling is not cystic also not moving with protrution of tongue. We recommended the patient for U.S. neck. It came as a ectopic thyroid in the midline and there was an abscence of a normal thyroid.
Then we took isotope scan, and surprisingly we saw another ectopic thyroid in the lingual area but it was very small. After the nuclear scan, we observed the posterior third of tongue with tongue depressor. We were able to see a small elevated ectopic thyoid which is a Lingual. We rarely come across this type of double placed ectopic thyroid. The patient was treated with Ltroxine 25 microgm & swellings are slowly subsiding.
