Spine surgeries under Spinal Anaesthesia — was a joke — For a long time. Even we condemned it many times, in POSTGRADUATION. But in Private practice — poorly equipped operation theatres, unqualified assistants, Poor patients, low packages, etc etc — Forced us to try Spinal Anaesthesia for Spine Surgeries. Initially, we started with short duration, safe cases. Later we did Prolonged cases UPTO 5 hrs. Dexmedetomidine 100- 200mcg INTRATHECAL is helping us a lot — due to its complete Lack of Respiratory depression.
Patients are very comfortable even after 4 hrs. Butorphanol 2-4mg etc can be used for 1-2 hr cases. Surprisingly, we found it very comfortable, safe, even in high risk 70 yr old patients — after doing many cases.
