Carcinoma of the esophagus is known to be in the advanced stage at the time of diagnosis and is also associated with transmural invasion and early lymphatic spread in most patients. 3-field lymphadenectomy has been widely practiced since the 1980s, with R0 resection as the aim of the surgery. Recent reports indicate improved 5-year survival and a reduction of the locoregional recurrence rate compared with the more classic 2-field lymphadenectomies.
