55y/F, k/c/o T2DM since 20 yrs, now only on insulin Human mixtard 30/70 (35 - o -35) from last 8 yrs (not on any OHA's), k/c/o HTN since 10 yrs taking Anti HTN, H/o of 2 angioplasty done first one in 2008 and second one in 2013. Now investigations done s/o Sr.creat 1.9, FBS 250, PPBS 410, A1C 10.8%, SGOT 58 SGPT 64, and deranged lipid prof, 2D echo not done. (wt. 60 kgs). Please suggest I am thinking of initiating Insugen (30/70) 25-0-25, tab. Gliclazide (80) 1-1-1, tab.
Trajenta(5) 1-0-0, tab. Voglibose (0.3) 1-1-1 and rest continue all other medications for HTN N Heart. Is that OK if I start Trajenta without knowing Report of 2D echo (LVEF)? Pt. is not having any symptoms of CCF clinically (No pedal oedema, no breathlessness) How safe is Trajenta in Post Angioplasty or Bypass patients? Please comment, do I need any modifications?
