Case Presentation: A 36-year-old male with no significant medical history presented to the emergency department with complaints of: Generalized fatigue and weakness Diffuse abdominal pain Weight loss of 5 kgs Laboratory findings: Hemoglobin - 8.8 g/dl Mean corpuscular volume (MCV) - 88 fl Thrombocytopenia Leukopenia Mild transaminitis. Vitamin B12 level - 100 pg/ml Iron -ย 56 mcg/dl Total iron-binding capacity (TIBC) - 291 mcg/dl Ferritin - 272 ng/ml Serum lactate dehydrogenase (LDH) - increased Serum haptoglobin - decreased The patient had elevated levels of urobilinogen levels in his urine sample. A computerized CT scan was performed.
