Case Presentation: A 56-year-old man was presented to the emergency department with complaints of mumbling incomprehensible sounds. The patient also showed symptoms of narrowing or blockage of arteries in the upper extremity. The patient had a history of atrial fibrillation, atherosclerosis, and factor V Leiden thrombophilia. A detailed examination revealed that he had some sensory deficits.

The area that was affected corresponded to the structure that takes up the most area on the sensory homunculus. The physician suspected that the middle cerebral artery was involved. Also, he planned carotid endarterectomy, a surgery to remove plaque.
