Advanced ALK-positive NSCLC treatment has shifted from the first-generation TKI crizotinib to second-generation TKIs. Although proven clinically beneficial, acquired resistance develops eventually leading to clinical relapse. Lorlatinib, a third-generation, oral, reversible TKI, penetrates the CNS and overcomes the secondary resistance mutations in the ALK tyrosine kinase domain. It has greater efficacy over standard therapy in patients with ALK mutations.
