73 yrs old doctor pt. Consulted orthopedic suegeon for giddyness fuzzy vision while stooping .in Nov.2013. MRI spines revealed cervical sponylitis BUT it also showed enlarged sella. Neurosurgeon advised MRI brain with sella in focus. It revealed Pitutary Macroadenoma of 14-16-20mm. The tumour was pressing on the chiasma but There were no pressure symptoms . Visual feilds were normal.
All hormones were normal except IGFi and testosterone. Kept under observation for one year But pt was stressed and life style disturbed due to this findings and persistence of giddyness or instability . Agreed to undergo micro surgery through nose in Oct.2014. Apfter six months MRI revealed a small size residual tumour. Visual feilds normal and hormones as before surgery, T3,T4 ,TSH, Cortisole all normal range AND IGFi ,testosterone still low as it was before surgery. Pt has lost 3 kg wt, he has still…
