Hypertrophic cardiomyopathy (HCM) is a genetic myocardial disease characterized by unexplained left ventricular hypertrophy and dynamic left ventricular outflow tract (LVOT) obstruction. While some patients have resting obstruction, many have latent LVOT obstruction that is detectable only with provocation testing. Because resting echocardiography may underestimate disease severity, appropriate provocation techniques—including the goal-directed Valsalva maneuver —play an important role in unmasking clinically significant gradients, supporting risk stratification, and guiding treatment decisions.
