A 70-year-old man brought in hospital emergency with h/o altered behavior and slurring of speech for last one day, before that he use to walk daily for about a 200-800mts in a local park, particular about his diet which contains good amount of fruits, vegetables, and milk ,he is vegetarian,left smoking and fatty meal 30 yrs back he is a social drinker in one or two occasions a month, no history of diabetes or hypertension and no history of intake of alcohol or recent head trauma prior this episode. On examination, afebrile, pulse78/min, bp recorded 130/60, respiratory rate was little high 24/min JVP not raised, CVS/CHEST/ABDOMEN examination unremarkable: CNS -drowsy but arousable, oriented to person but not to time and place, his GCS was M6 V3 E3 =12/15.
