A 55-year-old patient, a chronic heavy smoker with a known case of psoriasis since 20 years had an episode of fever and joint pains all over the body. Stiffness is more in the morning and subsides as the day progresses, since 4 years back, treated symptomatically this joint pain now non-progressive, but remains more in the morning and subsides as the day progresses. His blood counts are normal except low platelet count ranges from 20000 cells/cumm to 50000 with serum calcium 8.2mg/dl. On examination, we saw mild hepatomegaly.
