Case Presentation A 36-year-old male with a two-year history of persistent lower lip swelling and burning sensation was evaluated for chronic ulcerative and crusted lesions of the lower lip. History of Present Illness Initial appearance of two erythematous swellings on either side of the lower lip. Gradual progression to pain, severe burning sensation, ulcerations, crusting, and intermittent watery discharge. Symptoms persisted despite multiple courses of antibiotic therapy.

No history of lip biting, lip sucking, or known allergies. Occupational and Exposure History Occupation as a shepherd with prolonged and continuous exposure to sunlight, suggesting actinic influence.
