A 58 yr old male underwent coronary stenting in the backdrop of vascular risk factors like DM HTN DL for 2 vessel disease. On the fifth day, the patient developed altered behavior, levitation left UL readmitted. Initial CT showed small subcortical lacunes which couldn't explain his neurological condition.

He was on dual antiplatelets after stenting. His MRI is attached. What could be the possibilities from MRI and how do you go ahead in such situation?
