Case: A 55 year old gentlemen with no past history and no risk factors for immunocompromised state, presented with the complaints of being withdrawn from the surroundings, difficulty to speak, write, form complete sentences, remembering words since 3 months, fever since 2 days and one episode of seizure recently. On examination patient was conscious, oriented, vitals stable, with Mini Mental Score of 23/30. There was inability to form words, name objects, form complete sentences, speech was non fluent, with poor repetition. Cranial nerve exam revealed right facial nerve palsy.

Rest of the neurologic and other systemic exam was normal. MRI Brain revealed  space occupying lesion involving left Frontal lobe with marked perilesional edema extending to temporoparietal area. Based on the above findings he was started on anti tubercular therapy, antibiotics, steroids, anti epileptic therapy…
