A sixty yr old male presented initially with acute retension of urine and inability to walk. Urinary catheterisation was done and removed in 1 week. The patient then developed over a period of 4 months heaviness of both lower limbs/ numbness over both soles/ pain over lower limbs/ urinary urgency and frequency/ constipation and back pain.

Patient denies any motor/sensory/bladder or bowel disturbance in past.On examination power of 4/5 both lower limbs; DTR diminished; Probable diagnosis of transverse myelitis was made and sent for investigations. MRI showed : lumbar spondylotic changes; L4 -5 diffuse posterior and bilateral posterolateral disc bulge and osteophytes.This along with severe bilateral facet joint hypertrophy seen narrowing the spinal canal at this level and obliterating subarachnoid SCF spaces mildly narrowing bilateral inferior neural foraminae; Early bilateral facet joint…
