Endoscopic pituitary surgery has been the gold standard for treating micro-and macro-pituitary adenomas, including those with suprasellar and cavernous sinus extension. Although endoscopy enables a multi-angled view with excellent illumination and magnification, a surgeon must be thoroughly skilled to perform the surgery. Some of the important aspects include proper selection of cases, gradual transition from microscopic to endoscopic approach, adequate sphenoethmoidal recess widening, identification of important landmarks during each stage of surgery, and use of neuronavigation. A thorough knowledge in pituitary transposition techniques is essential to reduce complications.
