Achieving functional near vision without compromising distance acuity remains a key challenge in cataract surgery, even with modern extended depth of focus (EDOF) intraocular lenses. This study evaluated whether a mini-monovision strategy could enhance visual outcomes after bilateral implantation of Vivity EDOF IOLs. Sixty-one patients were assigned to either mini-monovision (non-dominant eye targeted at −0.5 to <−1.0 D; dominant eye emmetropia) or bilateral emmetropia.

At 1 month postoperatively, distance and intermediate visual acuity, objective visual quality metrics (MTF, Strehl ratio, aberrations), functional vision scores, and patient satisfaction were comparable between groups.
