A pt of 32 years of age presented with fever since 2 months. Cough with expectoration since 40 days and 12 kg wt loss. I have investigated her all the blood tests were normal except low Hb concentration and moderate increased ESR.

X-ray chest was done. But I was not able to understand the x-ray. Please any one suggest the dx.
