Can a single preoperative oral dose of clonidine sharpen the surgical field in endoscopic sinus surgery (ESS)? A recent systematic review and meta-analysis of eight randomized controlled trials (n = 597) has answered with compelling data. Pre-op clonidine significantly improved surgical field quality (MD = –0.65), reduced intraoperative blood loss by ~45 mL, and lowered MAP and heart rate—without raising postoperative risks. Surgeons also reported higher satisfaction scores, with Trial Sequential Analysis confirming conclusive evidence for most parameters.

These findings highlight clonidine’s dual role in enhancing visualization and stabilizing intraoperative hemodynamics, potentially redefining perioperative pharmacologic protocols in ESS.
