Fever for one month and a wide range of investigations and multiple doctors only could come to a possible diagnosis of adult onset still's disease (AOSD). AOSD may not be life threatening but the disability and pain of joints make the patient suffer. Differential diagnosis ranged from viral chikungunya, connective tissue disorders, rheumatoid arthritis and early onset of leukemia, the rheumatic factor being positive was the only clue to AOSD.
Where did we go wrong in not diagnosing it early? How common is AOSD and what is the prognosis?
