Case Presentation A 42-year-old male presented to a clinic with cough, shortness of breath, and fever. He had been experiencing these symptoms for the last three years. History He had received multiple treatments for rhinosinusitis in the last three years. Investigations A physical examination revealed inflamed nasal mucosa, nasal crusting, and saddle-nose deformity. Wheezes and rales could be heard upon auscultation.
Computed tomography of the chest and the face revealed multiple pulmonary nodules, and extensive destruction of the structural bones of the midface, respectively. This had resulted in the formation of a large nasal cavity (as seen in the cover image). The patient tested positive for proteinase 3 antineutrophil cytoplasmic antibody.
