A 52-year old male patient with lung and liver metastasis (1 st image) of malignant melanoma was advised to start nivolumab therapy (3mg/kg every 3 weeks). After approximately 34 weeks of nivolumab administration, a sharp increase in the liver enzymes (AST/ALT) (1225 /824 U/L) was noticed. However, the patient did not experience any symptoms. CT scans indicated the exacerbation of liver metastasis.

The lesion was localized, nondiffuse, but slightly larger (2 nd image, yellow arrows) than what it was before the therapy was initiated.
