A 31-year-old woman was presented with multiple necrotic skin lesions on her ears, nose, and legs that were developed over few days. Initially, lesions were erythematous-bluish macules which soon became bullous. Within 10 days, they had formed thick necrotic plaques, as indicated in the attached figure. This woman had no underlying diseases or infections, but she described exposure to severe cold while trekking in the mountains. Examination Histology revealed thrombosis of all dermal and subcutaneous vessels. But there was no inflammatory infiltration.

There were no signs of vasculitis. A test for cold agglutinins was positive and there were signs of mild hemolytic anemia.
