A 59-year old patient was presented with progressive disfiguring swelling around the neck and shoulder regions for the past 3 years. The patient was alcoholic. Examination The swellings were subcutaneous, doughy, and nontender. Lips, face, and legs remain unaffected. Biochemical parameters were within normal range.

Fine needle aspiration indicated lipomatous lesion. Other physical attributes included buffalo hump, pseudo-athletic appearance, and horse collar (as from the attached images).
