Inability to write in 10 years old Child

A 10 yrs old boy's complained of not able to write since child hood, his mother says he understands and remeber every thing but not able to write. He has one more problem when he walk he used to collide with any thing that comes on the way i.e.

if there are bricks lying on the the way he will collide with it  and he will not change his path to avoid collision. His vision is normal.
