I used Dexmedetomidine intrathecal up to 200 micrograms. I did 100 cases comprising of upper abdominal surgeries, Bil knee, Hip replacements, Bil pheochromocytomas etc. I used this sometimes as an adjuvant to general anesthesia, especially in renal failure patients (posted for Ectopic thoracic parathyroid surgery with sternotomy). It's very safe very good anesthesia up to 8 hrs. No side effects encountered. There is an initial rise in blood pressure but later controlled hypotension.
Bradycardia easily handled with atropine/hyoscine . Patients are very happy with this intrathecal use. All the International/ National studies to date were with 20 micrograms only. But 200 micrograms can be used safely intrathecally. Share your views.
