A 24 years old woman, delivered a premature baby of 7 months, during the course of pregnancy she had continuously cough and dyspnoea problem. After two of delivery she is feeling extremely tired, fever, anorexia, giddiness. X-ray chest posted, is OK. Effusion Lt side, with milliary/fine noduloreticular shadow both lungs.

CBC normal, ESR 55mm. Sputum for AFB grade 1 positive. No response from ATT, the patients condition is worsening. How to treat her and investigate?
