Sudden onset of high fever after root canal treatment. Most of the joints were swollen and tender. Maculopapular rash appeared on the third day followed by conjunctivitis and scotomas in the eyes.

Crepitations appeared in the mid-zone part of the right lung. All laboratory tests were negative except for raised CRP and SGO. What is the diagnosis?
