It is essential that in any procedure in OT or in Any lab, we should disclose the names of all doctors operating, assisting, anesthetist, nurses and paramedical staffs etc in post OT or lab note book beside writing details of procedure with time taken, any difficulty noted or any improvement or complication happened or may likely happen. We are bound to give it to patient or it's attendant wanting for knowledge or file a case in consumer or other court . If we ate not disclosing the names of doctors in the surgical team, it may be deficiency of service.
