A 50 years male patient admitted with c/o of severe anal spasm, with severe pain during defecation. There was a chronic fissure at 6 with intersphincteric fistula with rectal perforation at 7 O' clock.

Laser treatment for fistula done, approx 600 joules spent. Patient discharged in 24 hours.
