Many of our fellow doctors do not write the diagnosis on their prescriptions. Is it not mandatory? Is it unethical? Is it illegal? Is it against the MCI rules?

Can such prescriptions challenged in the court of law? Even some are in a habit of not writing the name of patient,age, sex, date  and signatures on their prescriptions. What is the opinion of our group members regarding it? kindly share
