A 55-year-old female known case of chronic lower limb DVT presented with Osteosarcoma of left maxilla involving orbit. She had a history of trauma to left side of face and forehead for the same STG was done 5 years ago. In view of locally advanced disease, Neoadjuvant Chemotherapy and preoperatively Radiation was given. Partial response was achieved.

As a patient was a knwon case of DVT, and prior history of trauma to face as forehead flap could not be used. It was challenging to do. Microvascular reconstruction. We did Total maxillectomy with enucleation followed by Microvascular Reconstruction using Anterolateral thigh flap (Brown Type 4a defect).
