A 36 year old lady weighing 75 kg with h/O Dm for eleven years and hypothyroidism for nine years {both under control} underwent T.A.H. for fibroid uterus two months ago. Routine investigations were normal. SA was given with 25 G Quinke needle with 2.8 ml of 0.5% bupivavain. Surgery lasted 45 mins. Patient did not recover motor and sensory loss post op, but developed weakness of upper limbs after 12 hours which subsided after six hours.

MRI showed myelitis of the whole spine with no signs of injury due to the spinal needle. Pt was treated with methyl prednisolone for four weeks  showed no recovery. five doses of Igg was given. No improvement, but developed quadriplegic. I wish to know the cause, with references as this is under litigation.
