A patient came with pain and mild tenderness RLQ and right paraumblical region, a 40 years female, patient was admitted for observation had no convincing evidence of appendicitis no marked tenderness or gurading mild leucocytosis but did not improve so was taken for appendectomy, caecum was little high up needed mobilisation and appendix was found normal pearly white and not inflammed but patient was mildly obese with friable mesentery and manipulation resulted in mesenteric tear behind terminal ileum which in attempts to suture and ligate kept extending in a mostly linear fashion for 4-5 cm but was controlled but it could be seen the terminal ileum becoming a bit congested but it was ignored and thought it will resolve as it had happened in some of earlier cases also and nothing happened but this patient had an uneventful recovery and normally.
