Dictionary meaning of the word doctor is "A person with a medical degree or qualified to treat people who are ill or hurt."  It does not specify degree or required qualification. In ancient times, the word doctor was restrictedly used for elderly religious people of the church. It was a word which gave the highest respect to guides or mentors of humanity. Later on, when the first university was established in medieval times in Europe, this term was also sparingly used for highly qualified and experienced men in late stages of their lives.

Gradually, it became a tradition to address people, a doctor who achieved something worthwhile in their own subject or pursuit.
