A 49 years old man has diabetes mellitus following acute pancreatitis due to chronic alcoholism. He was prescribed insulin initially followed by oral antidiabetic agents. There is the history of injuries and hospitalisation due to severe hypoglycemia on several occasions and sometimes fall in blood sugar is abrupt in nature. One morning, FBS was 110 mg %, immediately after breakfast patient complained of tremors of hand, weakness, low energy levels, blood sugar tested by glucometer was 100 mg %.
