A giant bulla is a complication of emphysema. As an air pocket, a bulla grows, it takes up space in the chest cavity and can encroach on the lungs. Severe disruption of normal lung function due to the crowding and pressure is a common result. This case highlights a procedure namely Segmentectomy of a chronic smoker with complaints of progressive breathlessness on exertion; in radiological examinations, a large bulla in right lower lobe of lung was revealed.

Therefore, VATS Bullectomy and Pleurodesis was planned. The presence of bulla in the pleural cavity, superior segmented bronchus was dissected, looped and divided. All the steps of the surgery are shown in detail. What are the challenges that you have faced during segmentectomy?
