Case History: The deceased was a pillion rider who had sustained injuries when the motorcycle collided with an auto-rickshaw. He was treated at a nursing home as out-patient. Approximately after 3 weeks around 2 am in the night the deceased started complaining of breathlessness and burning sensation in epigastria with profuse sweating. Later, the patient was treated at local nursing home and was referred to a multispecialty hospital.

Past and drug history were not contributory. Course at Hospital: Patient presented with the above-mentioned symptoms was immediately shifted to CCU. Investigations performed include: ECG showed Q S in lead II T wave inversion in lead III Chest leads V 4-5. Screening ECHO was done at the bedside and revealed dilated right atria and right ventricle, severe right ventricular dysfunction, a big clot in right atria proposing into right ventricle with fair LV…
