A 28-year lady receptionist of an office presented with a tremor of both hands causing extreme difficulty in operating computer, making her compromised in doing her daily works for last 3 months. Along with she also complains of depression and some amount of forgetfulness regarding the recent memory and recalls are concerned. She took consultation of a local doctor and consulted a psychiatrist also who gave her some combination therapy with Propanolol 10MG BD ( beta blocker) and desvenlafaxine -an antidepressant.

She got some relief of tremor but developed difficulty in walking fast with high heel shoes and some imbalance also during walking. Her routine blood checks up, lipid profile, liver function tests and renal function tests unremarkable except marginally elevated liver enzymes.
